NEW CLIENT FORM

• Please click above link to submit a NEW CLIENT FORM on Dr. Browns main site (Brownvetservices.com) and follow outlined scheduling protocol 

​

• Please have medical records emailed to soulpuppyhealing@gmail.com directly from your pets current/previous veterinary hospital/clinic(s)

​

Once forms and records are received, Dr. Browns online assistant, Deb, will be in contact regarding request

​​

• Dr. Brown is at Soul Puppy by scheduled appointment only 

​

• Due to the nature of this practice the following policies are in place:  

  • $50 minimum deposit is required to hold/confirm appointments which will be deducted from total invoice​

  • 24 hour $50 cancellation fee

  • $100 no show fee​

  • Aggressive patients or patients with a history of needing sedation for exams will not be accepted for safety purposes​

  • Masks continue to be required due to Covid-19 and its variants. 

  • Please do not bring additional pets to your scheduled appointment. One adult per pet will be allowed into office due to the size and location of Soul Puppy's healing space.
