STEP ONE
Please complete the required online new client request form above and immediately go to step two

MEDICAL RECORD RELEASE
STEP TWO
Please release full medical records including test results and rabies & vaccine information to Dr. Brown

IMPORTANT: Pet owners must call veterinary offices directly to release & request records be emailed to records@brownvetservices.com. Requests are held at this step until records received & reviewed for safety purposes.
Failure to provide records within 14 days of form submission will automatically deactivate request.

Dr. Brown does not accept pets with a history of aggression or in need of sedation for exam. Probationary acceptance considered on a case by case basis.

DEPOSIT FOR APPROVED REQUESTS
STEP THREE
Approved requests will be emailed instructions on how to submit the required deposit before proceeding to scheduling.

Approved requests will have 72 hours to submit the required deposit before being automatically deactivated.
​
*If your request is not approved a courtesy email will be sent with additional information

IMPORTANT: submitting unauthorized deposits violated Dr. Browns Zero Tolerance Client Aggressive Behavior and Bulling Policy


APPOINTMENT OFFERINGS
STEP FOUR
Preapproved request with authorized paid deposits will be emailed/texted the first available appointment option(s)

Appointment options are held for 24 hours before being automatically offered to the next patient on the wait list.

Deposits will be deducted from invoice total.
​
A scheduling date must be chosen within 30 days of deposit submission or the deposit will be forfeited and the request deactivated.
Dr. Brown reserves the right to refuse service at her discretion
