STEP ONE
Please complete the required online new client request form above and immediately go to step two

Note: all required fields must be filled for forms to process correctly. If you do not receive an email from scheduling within 1 to 3 business days acknowledging receipt of your request then we did not receive it or the email provided on the form had an error. IMPORTANT: Please respond to emails for best service and to keep request active. Please submit any questions by replying directly from our email vs starting new threads for tracking purposes.

MEDICAL RECORD RELEASE
STEP TWO
Please release full medical records including test results and rabies & vaccine information to Dr. Brown

IMPORTANT: Pet owners must call pertinent veterinary offices directly and request records be emailed to Dr. Brown at records@brownvetservices.com. Requests are held at this step until records received for safety purposes. Dr. Brown does not accept pets with a history of aggression or in need of sedation for exam.

DEPOSIT FOR APPROVED REQUESTS
STEP THREE
Approved requests will be emailed instructions on how to submit the required deposit before proceeding to scheduling

APPOINTMENT OFFERINGS
STEP FOUR
Preapproved request with authorized paid deposits will be emailed the first three available appointment options

IMPORTANT: appointment options are held for 24 hours before being automatically offered to the next patient on the wait list. Deposits will be deducted from invoice total or refunded if offerings are not accepted. Dr. Brown reserves the right to refuse service at her discretion
